1. Is there anything I need to do prior to my appointment

Yes, all potential patients are required to complete an online Health History form

If you have been seen by one of our providers in an office prior to 4 Better Health, yes, please fill out the health history form and all required health forms.  

If you have been seen by one of our providers in our Hopkinton office and it has been less than 18 months, you do not need to fill out a health history form.

If you have been seen by one of our providers in our Hopkinton office and it has been over 18 months, you do need to fill out our returning patients forms.

Once Health History forms are submitted, our providers will review the completed form and contact the patient within 7 business days. Not all patients and providers are a match. Once a decision is made, our admin will reach out to set up the initial consultation or to offer other practice recommendations.

No, we are not primary care providers. We are a specialty Functional Medicine practice and require that you have a PCP to work with us.

Yes. We see men and women over 13 years of age.

No, none of our providers are licensed for patients younger than 13 years of age.

No, we are a fee-for-service practice. We are not a participating provider for Medicare or Medicaid. Our office does not work with insurance providers or referrals. Read more about why we don’t take insurance.

Investing in your health is not a one-shot deal. It’s a journey. Most of our patients will need 2-3 visits in the first 3 months and then have check-ins 2-3 times yearly. The first appointment is typically 1.5-2 hours ($630), follow up appointments range from 30-60min depending on complexity ($230-400). Your health investment often includes copays for functional medicine testing as well as possible supplementation. Many of our patients will say “It was the best $1000 I ever spent!”. Health, Vitality, Longevity and Energy are lifelong gifts for you and your loved ones. If you have the opportunity to put money aside into a flex spending or HSA card, please do as we do accept these as well (see #11)

Yes, we accept all forms of credit.

Yes, we have payment plan options.

Yes. You will receive a receipt for service in the form of a “Superbill” with all required CPT and ICD10 codes. Please keep this for your records should your insurance, flex or HSA require this. While we accept these cards for payment, we cannot guarantee your HSA or Flex will accept our Superbills.

Yes, we do offer HIPAA compliant Phone and Telehealth appointments as long as we have seen the patient in our office for an initial consultation. Patients are required to be seen in the office at least once yearly. We offer Telehealth only appointments for Florida residents without the need to be seen in our office.

Yes, for your safety and ours, medically legally we are bound to offer top care with due diligence with a full appointment which may include a limited physical exam.

No, the office is open by appointment only. Office hours are Monday-Thursday 9a-430p. Appointments are scheduled 9am-noon and 1-3p. We are closed 12-1. We are closed on Fridays for office appointments, however, our IV suite is open Fridays and two Saturdays/month.

Yes, we have strict 3 Business Day cancellation policy.  All of our policies can be found on “Our Policies” page.

Preparing for your Initial appointment

Prior to coming for your initial appointment, we ask that you follow the following steps:

Collect Labs and/or Imaging reports

Our practitioners request that you work to attain PDF copies of your latest labs and pertinent imaging reports (scopes, MRI’s, CT scans, ultrasounds, etc.). We request these be uploaded to our office at least 72h prior to your initial consultation by emailing them to Info@4BetterHealthMedicine.com.

We do not have the time to stop and review your online medical records, nor does ‘sharing’ your record work due to password restrictions.

Call your Insurance Company

Yes, most patients do.  Depending on the insurance plan, however coverage is highly dependent on your plan and deductible. It is impossible for any medical practice to know what each unique insurance plan covers and or carries for deductibles. We request all patients call their insurance prior to their first appointment and before having any labs drawn to inquire about deductibles, lab and/or ordering practitioner restrictions:

  1. Is it ok if your labs are ordered by an out-of-network provider?
  2. Do they allow you to go to Quest?
  3. What is your deductible for labs (and how close are you to meeting the deductible at this point)?

What should I bring to my appointment?

Please bring your supplements and medications. If you did not have a chance to upload lab or imaging reports, please bring copies of these.  Feel free to bring a friend or family member to offer another set of ears for your recommendations moving forward.

Payment

Payment by cash, check, card, flex or HSA is expected at the time of service. All return checks are subject to a $35 fee. Any refund request will be processed minus processing and banking fees.

All balances over 60 days are subject to interest of 1.5% per month. We reserve the right to apply a $25 rebilling fee and $25 late charge for any payments overdue more than 14 days. Any accounts over 90 days past due will be required to pay to include all late fees prior to scheduling a future appointment.

Will I receive a reminder for my appointment?

YES.  Our automated system sends an email reminder the week before your appointment, a phone reminder the day before your appointment and a text reminder the day of your appointment.

Upon Arrival

You will have your vital signs taken and an InBody composition scan done.

Consultation With Provider

Initial appointments are 1.5-2 hours in length.  Follow ups are 30-60 min in length depending on complexity. We recommend you eat prior to your appointment and bring a beverage.

Labwork

No labwork will be done at your initial appointment. Your provider will discuss recommendations with you after listening to your unique health history and goals. Lab kits or orders will be given at the end of your appointment.

Supplement Recommendations

Your provider may recommend change or stop current supplements and/or recommend new ones. This will be discussed and printed at the end of your appointment.

Portal Access

Our team does our best to communicate with our patients through the HIPAA compliant portal. We ask all patients to complete their portal registration within 48h of their initial consultation. This allows your provider to communicate with you privately, easily and with medical record documentation.

We use encrypted email for any communication that includes personal identifiable information. Through these services patients will also receive confirmations and reminders. Patients have the option to opt out of these communications, but we do not recommend it. Doing so will remove the patient from ALL notifications, and you will miss reminders and recommendations.
